Arrival and departure permission

At Neutral Bay OOSH our Arrival and Departure Policy requires children to be signed in and out upon arrival and departure by parents/guardians or other authorised person.

If a family would like to grant permission for their child to arrive or depart unaccompanied, expressed written permission must be provided via this form and families must confirm that they understand the conditions of such permission. This permission form also relates to situations where a child will be collected by a person under the age of 18 years old.

By completing this permission form you understand:

  • Parents must instruct their children to see the educator leader upon arrival and departure to be signed in/out.

  • Children arriving unaccompanied must arrive by 8.30am. If your child has not been signed in by 8.30am they may be marked as absent and a CCS absence may apply.

  • All responsibility for the child's health, wellbeing and safety is that of the parent/guardians until the child is signed in and thereafter once signed out.

  • It is the parent/guardian's responsibility to assess the child's suitability to arrive/depart unaccompanied by an authorised adult.

  • Our Electronic Sign In procedures provide notifications to parents about incident forms, absences and confirming attendance records. If families do not sign in/out their child because their child is arriving/ departing unattended, it is the parents/guardians responsibility to track the relevant information through the HubHello portal.

    *In the case of serious injuries or incidents families will be notified via alternate means as well (eg phone call, text message, email depending on the nature of incident).
